From what we can tell so far, the upgrade system on PS5 will run exclusively through digital downloads. Regardless of what version of the PS4 game you own. This means that even if you buy the physical PS4 version of Assassin's Creed: Valhalla, for instance, you'll have to enter the disk into the PS5 and download the PS5 version digitally. An important thing to realise here is that, in order to receive your PS5 upgrade of a physical PS4 game you will need to own a disk drive edition of the PS5. The digital only console will not allow any upgrades via a physical disk.
On Xbox this process seems to be a little easier by simply including both versions of the game on the physical edition, you'll be able to simply enter the disk into either the Xbox One or the Xbox Series X/S and play the appropriate version for that console.
